WARNING: This is a big theory!
First of all, John Watson cannot see Sherlock actually fall to the ground because he is behind a wall.
Secondly, John Watson is distracted and delayed by a bike, this gives Sherlock the advantage to LAND ON THE TRUACK WITH THE PILLOWS ON and then sneak away whilst a dummy Sherlock is dropped from a high window from the hospital.
That is my theory.
